Floating support

  

Our Floating Support team assists adults living in their own home to carry out their housing responsibilities as independently as possible by tailoring a programme to suit their individual needs.

All services are focused on the goals of the person and based on partnership working with them, their families and other health professionals.

The programme is tailored to suit each individual. Time is spent initially to design and agree a personal Support Plan. Typically support is up to 6 hours per week over 6 - 9 months. Support covers things like:

Support is short term and does not include personal care support but is ideal if you require advice to help you have a more independent, less chaotic lifestyle.

Referrals can be made by anyone interested in accessing the service for themselves or by a professional, family member or other organisation.
