David and Floating Support Worker, Clare, have been a dedicated team for 22 months. Their collaboration has been marked by a significant amount of support identified at initial meetings, with a number of further goals added throughout David's time in the service. Clare has noted that this is the most comprehensive support she has provided to a single service user.

Image shows David in his home smiling at camera.David and Clare met with the NI Housing Executive (NIHE) to request repairs and maintenance at David's property. This included providing evidence to NIHE for David's need for an automatic door installed at the front of the property and for flooring throughout the property to be replaced. Through the collaboration of David and Clare, NIHE completed all repairs and maintenance. This was a significant achievement, a large piece of support that took David and Clare over a year to complete, and it brought David a sense of relief.

David works hard to be as independent as possible but faces barriers, including safety concerns, when he is out of the house on his own. Clare provided David with information on the Radius Connect 24 smartwatch and personal alarm service, which works inside the home and when David is out. Clare applied for a grant with Independence at Home to fund the smartwatch and the first year's subscription to Radius Connect 24. This service has proved to be a lifeline for David, providing him with a sense of security and peace of mind. He has used the watch several times when he has required assistance from emergency services.

Clare also made referrals to other services for David. David is still involved with Cedar's Inclusion Works Service 'which will undoubtedly help David find employability further down the line' and has had support from RNIB. Clare accompanied David to the Men's Shed services in Coleraine and Portstewart. 

Clare supported David in renewing his Blue Badge and applying for an electoral ID card so he would have a form of photographic ID.

Clare referred David for a benefits check and had several discussions about budget management. She also supported David with his application for a Bryson energy grant, and David was given a £200.00 top-up of electricity just before Christmas 2023, and a Keep Warm pack from the local council was also allocated to him.

David expresses, 'I deeply appreciate Clare's unwavering support, which has significantly improved my life. I am saddened that our journey together has come to an end.'

Clare reflects, 'I have reminded David of our successful journey together, where we worked tirelessly to achieve his goals. It's a testament to our hard work and dedication.'
