Transitions Service
Transition is a term commonly used to describe the process that a young person experiences as they move from childhood into young adulthood. This is a time of considerable change for each young person and their family involving important decisions and life choices with regard to further education, training, employment, independent living and citizenship.
The Service is delivered through a dedicated team of Transitions Officers. The Transitions Officers will supportyoung people to achieve their maximum potential and ensure they participate in all aspects of community life.
The Transitions Service is delivered in partnership with the local Health and Social Care Trust and is committed to:
- Ensuring a person centred approach to the transitional planning process and providing support to young people at key times in their lives.
- Delivering a transitions service based on interagency partnership and collaboration.
- Identifying young peoples aspirations and ambitions for life as early as possible to allow for planning and assessment.
- Providing young people with equal access to locally based services including education, training, employment and recreational.
- Supporting young people to achieve their full potential in adult life by ensuring they have access to a wide range of services i.e. post school options, Careers Service, Health & Social Services….
- Highlighting the young person’s support needs and make certain these needs are addressed in their chosen post-school provision.
- Monitoring young people’s progress in their new provision and providing on-going support whilst on the Service.
- Actively promoting social inclusion and encouraging young people and their parents/carers to make informed choices.
- Providing a range of social and personal development activities for young people to participate in, which will enhance their personal and social life skills.
