Transitions Service
The Transitions Service aims to support young disabled people through the transitions process. The service is delivered through a dedicated team of Transitions Officers.
Aims of The Transitions Service
- To adopt an inter-agency co-ordinated approach to transitional planning
- To ensure a person centred approach to Transitions
- To liaise & plan with young people re: options after leaving school
- To initiate contact with parents and ensure they are part of the transitions process
- To work with schools to identify young person's further options and support need
- To network with relevant professionals and develop a multi-disciplinary approach to transitions
- To ensure that young persons support needs are addressed within their chosen provision
- To promote choice of opportunity to young people leaving school
- To support schools in ensuring that young people are given on opportunity to access work placement
- To highlight gaps in existing provision and work with other agencies in developing new services
- To provide an independent training programme to support young disabled people develop skills to assist in their transition
