Inreach
The Cedar Foundation Inreach Project, funded by the Big Lottery for 3 years, is an action research project which will provide a flexible family support service to children with complex health care needs and their families in their own homes. Cedar will deliver the service in partnership with the Belfast Health and Social Care Trust. There are 154 hours of support available weekly to families living within the Trust area.
The main objectives of Inreach include:
- Providing a Family Support Service to respond to the individual needs of children with complex needs and their family. Hours will be allocated on a weekly or fortnightly basis as agreed by the Paediatrician/Community Children’s Nurse(CCN)
- Working in partnership with the Belfast Health and Social Care Trust to provide a quality service offering a high standard of care for children with complex needs and their families. The Trust will refer families and will provide training for In-reach staff on supporting the specific needs of the child
- Providing parents with time out for other siblings and themselves thus promoting whole family well-being
- Disseminating the model of practice to other Health and Social Care Trusts within NI in order to benefit a larger population of children with complex needs regionally.
Referral Criteria:
To be eligible for the service children must:
- have complex health care needs and be referred by a Paediatrician or a CCN
- be on the case load of a Community Children’s Nursing Team
- be aged 0-18 years and living within the Belfast Health and Social Care Trust area
