Active Futures Programme
The Active Futures Programme in partnership with The Northern Health and Social Care Trust supports young people in the Causeway area with physical and/or sensory disabilities to ensure a smooth transition from school into employment, further education and community based programmes across a range of social, recreational, voluntary, vocational and educational sectors.
The Active Futures Transitions Programme (16-18 years) aims to :
- Ensure a person centred approach to Transitions.
- Liaise & plan with young people their options after leaving school.
- Initiate contact with parents and ensure they are part of the transitions process.
- Work with schools to identify young persons further options & support needs within their chosen provision.
- Promote choice of opportunity to young people leaving school.
- Network with relevant professionals and develop a multi agency approach to transitions.
- Provide an independent training programme to support young people leaving school.
The Active Futures Community Inclusion Programme (18 -25 years) aims to:
- Promote person centred planning methods and approaches including personal networks and contacts
- Involvement in mainstream educational training activities with the opportunity to gain qualifications
- Assist participants to access sustainable, socially inclusive activities in their local community
- Facilitate work placements with job coaching support to enhance opportunities to gain employment
- Provide an independent training programme to support young disabled people to develop skills to facilitate community inclusion
